Submission of the Membership Application is to include the US figure Skating Release Form and the Athlete Information Form. Please include the Application checklist and include all items requested.
Skaters competing at Puerto Rican Nationals and ISU competitions are required to submit the attached form; "Skates program content" for both the short and long skate program. This is for the "Planned" program content. This aids the Technical Specialist (caller) and the judges to know what elements the skater plans to perform."
